A Centre for Children and Young People with Disabilities

Dynamic Futures Project

Empowering young people with disabilities aged 15-25 years living in Wrexham to make a difference in their own lives to achieve their aspirations

Dynamic Futures is a young-person-led project funded by The National Lottery Community Fund to support young people with disabilities aged 15 – 25 years in Wrexham County Borough.

The project aims to build independent life skills, self-esteem and self-confidence along with emotional and mental resilience so that young people can successfully navigate the difficult journey from child to adulthood.

We have spoken with young people about the new project and they and staff describe it as:

‘Building Dynamic Futures Together’
The project that helps me be the best I can possibly be

Dynamic Futures can support any young person who meets the following criteria:

  • Lives in Wrexham
  • Is aged 15 (Year 11) – 25 Years
  • Has a mild or moderate diagnosed disability or learning disability;
  • Or, has several diagnosed learning difficulties that impact significantly on their daily life
  • Not be working with any other transition service

If the young person meets all the eligibility criteria, the next steps would be to make a referral. We are happy to receive self-referrals, referrals from parents/ carers or a family member or from professionals. Our referral form is available by downloading the form below.

Once your referral is received, it is reviewed by a panel made up of the Dynamic Futures Team and a Dynamic Trustee to ensure that Dynamic Futures is the most appropriate source of support for you/ your young person. All young people and families are contacted following the referral panel to share the outcome of the meeting.
